VIBEREAS, the City Council added a cafeteria plan in October 1990 to the list of benefits
provided to City employees;
WEEREAS, the City Council has previously approved the expansion of the cafeteria plan
to include dependant care, term life insurance and health care expenses; and
WHEREAS, the City Council has found that this action implements Finance Policy 1 of
the Century Plan - Policy Plan Element, and is not found to be inconsistent or in conflict with
any other Century Plan Policies, as required by Section 2.03 of the Administrative Chapter of the
Policy Plan;
NOW, THE, REF ORE, BE IT RE SOLVE D BY THE CITY COUNCIL OF THj
CITY OF GEORGE TOWN, TEXAS, THAT:
SECTION 1. The facts and recitations contained in the preamble of this resolution are herell
found and declared to be true and correct, and are incorporated by reference herein and express'
made a part hereof, as if copied verbatim. i
SECTION 2. The Mayor is hereby authorized to execute Supplements 4 and 5 to the City of
Georgetown Employee Cafeteria Plan, and the City Secretary to attest. 11 \—
SECTION 3. This resolution shall be effective immediately upon adoption.

Term Life Insurance Premium Payment Benefits Under Cafeteria Plan
1. Purpose. The purpose of this Supplement is to permit Participants to pay the
Employee portion of the cost of coverage under the Term Life Insurance Plan by making salary
reduction contributions to the Plan. This Supplement constitutes the term life insurance premium
payment plan of the City of Georgetown. The benefit provided under this Supplement is a
11premium payment benefit." The specific provisions of the Tenn Life Insurance Plan as set forth
in a contract(s) or policy(s) issued by a company shall be considered a part of the plan and
incorporated herein by reference.
2. Amount of Premium Payment Benefit. Subject to the conditions and limitations
of the Plan, each year each Participant may elect in writing on a form filed with the Plan
Administrator on or before the date he or she first becomes eligible to participate in the Plan, and
on or before the first day of any plan year thereafter, to reduce his or her compensation and to
contribute the amount of such salary reduction to the plan as his or her term life insurance
premium cost. The amount of term life insurance included in the Plan may not exceed FIFTY
THOUSAND AND NO/100 DOLLARS ($50,000.00). The terms "term life insurance cost," as
used in the Plan, means the Participant's share of the cost of term life insurance group coverage.
Provider. The provider of coverage under the Term Life Plan shall be Blue
Cross/Blue Shield of Texas, Inc. until such time as the City Council of the City of Georgetown
terminates or amends such coverage.
IN WITNESS WHE REOF, the City of Georgetown, having received approval of its City
Council, has caused this Supplement to be executed in its name and behalf this 14th day of
January, 1992, by its Mayor, thereunto duly authorized.

1.1 PuLpose. The purpose of this Health Expense Reimbursement Plan is to provide
for the reimbursement of certain qualified health expenses to Participants. This supplement
constitutes the health expense reimbursement plan of the City of Georgetown. The benefit
provided under this Supplement is a "reimbursement benefit."
1.2 Effective Date. The Health Expense Reimbursement Plan is effective as of
October 1, 1991.
1.3 Plan Administration, Plan Year. The Health Expense Reimbursement Plan
administered by the City of Georgetown on the basis of a plan year as identified in Article I
1
of the City of Georgetown Employee Cafeteria Plan.

Contributions
For Contributions provisions, see Article IV, Section 4.10 of the City of Georgetown
Employee Cafeteria Plan. However the maximum amount allowed Participant contribution per
plan year will not exceed ONE THOUSAND ONE HUNDRED AND NO/100'S ($1100.00) per
plan year.

General Provisions
1. Amount of Benefit. Subject to the conditions and limitations of the Plan, each
year each Participant may elect in writing on a form filed with the Plan Administrator on or
before the date he first becomes eligible to participate in the Plan, and on or before the first day
of any plan year thereafter, to be reimbursed for the City the health expense costs (as defined in
paragraph 2) incurred during that year by him or her to the extent that such costs do not exceed
an amount equal to the lessor of:
(a) The total qualified health expense costs paid by the
Participant and attributable to health care services
rendered during the plan year and regardless of
whether such cost are paid by the Participant during
such plan year; or

,,b) the amount allocated by the Participant to this benefit.
2. Costs. The term "health expense costs," as used in the plan, means amounts paid
by a Participant for non -covered health-related expenses that meet the criteria for deductibility
as medical expenses under Internal Revenue Code Section 213(e) including, but not limited to,
noncovered medical expenses, vision and hearing expenses, dental expenses not covered under
a current dental plan, and noncovered prescrition drug expenses.
3. Manner of Makinlz Payments. As of the pay date of each month, the City shall
reimburse each Participant for the portion of health expense costs that is payable to him under
paragraph 1, provided that the Plan Administrator receives a signed statement for the Participant
that he or she has not and will not in the future deduct such costs as expenses on his or her
individual Federal or state income tax returns. Reimbursement for qualified expenses is limited
to the account balance of deductions reduced by any prior reimbursements.
4 Non -duplication of Beneflts. A participant shall not be reimbursed for heal
expense costs under this Plan to the extent that such costs are paid to or for the benefit
11'articipant, or to or for the benefit of any individual included in his or her family unit, under tHe
provision of any other plan. I
