2.5 ADA Grievance Procedure
The City of Georgetown has established a grievance procedure for prompt and equitable
resolution of complaints alleging discrimination on the basis of disability in the provision of City
services, activities, or programs in violation of Title II. Complaints shall be in writing. Alternative
means of filing a complaint to accommodate a person with a disability, such as a personal
interview or a tape recording, will be made available on request. Complaints shall contain at a
minimum the following information:
The name, address and telephone number of the person making the complaint;
The date and time of the incident or condition observed or experienced;
A statement in sufficient detail to define and explain the incident or condition
and the alleged violation;
Suggested recommendations to adequately resolve the incident or condition
and the alleged violation; and
Any other written information or documentation (such as photos, maps,
diagrams) to completely explain the incident or condition and the alleged
violation.
A complaint form is available on the City’s Accessibility web page (ada.georgetown.org) and is
attached to this Transition Plan in Appendix A. A complaint should be submitted as soon as
possible but no later than 60 calendar days after the incident or condition observed as an alleged
violation. The complaint should be sent to the ADA Coordinator.
Within 15 calendar days after receipt of a complaint, the ADA Coordinator, or their designee
will acknowledge receipt of the complaint and may, at his/her discretion, meet with the
complainant to discuss the complaint. Within 30 15 calendar days after receipt of a
complaintthe meeting, the ADA Coordinator will respond to the complaint in writing or, if
requested, in an alternative format such as large print or audio tape, to accommodate a
disability. The response will explain the position of the City and offer options for substantive
resolution of the complaint, or advise the complainant that additional time is necessary to
provide a complete response.
If the options offered by the ADA Coordinator do not satisfactorily resolve the complaint, the
complainant may file an appeal with the City Manager within 15 calendar days after receipt of
the ADA Coordinator’s final response. The appeal should be in writing and sent to:

Within 15 calendar days after receipt of the written appeal, the City Manager, or designee, will
acknowledge receipt of the appeal and may, at his/her discretion, meet with the complainant to
discuss the complaint and the ADA Coordinator’s response. Within 30 15 calendar days after

accommodate a disability.
Written complaints received by the ADA Coordinator, appeals to the City Manager, and written
responses to complaints will be retained by the City for a period of at least three years from the
date of final resolution of the complaint or the date of the last written response from the City.

Waggoner informed the Board that he had a discussion with ??? of the ARCIL and
she is very willing to come and give the City some training.
G. Discussion of ADA Board Chair presentation to the Georgetown Commission on
Aging – Nat Waggoner, Transportation Analyst
Waggoner informed the Board that they had been asked to come and speak to the
Commission on Aging. The Chair and any other members are welcome to attend.
The Meeting will be held on Monday June 20, 2016 at 6:00 PM at the Public Safety
Building located at 3500 DB Wood Road.

letter of s uppo rt fo r F aith in Ac tion’s applic atio n fo r F TA 5310 fo r FY 2017. - Nat Waggo ner
ITEM SUMMARY:
CAMPO is now ac cepting applic ations for the Federal Trans it Ad ministratio n's Enhanced Mobility for
Seniors and Indivi duals w ith Disabilities (S ec tio n 5310) program. The federal fund ing availab le for this
call fo r projec ts in our regio n is approximately $1.33M.
The grant p rovid es funds to s erve the spec ial needs o f transit-d ep endent populations beyo nd traditio nal
p ublic transpo rtatio n s ervic es and Americ an with Dis ab ilities Act (ADA) c omplementary paratransit
s ervic es .
Faith in Action (F IAG), through assistanc e fro m the City, has s ecured FY 16 monies (approx. $53K) from
Capital Metro as well as the required match fro m the Geo rgeto wn Health Foundatio n. F aith in Ac tion will
b e us ing the F Y16 mo ney to build ride s cheduling c ap acity.
F aith in Ac tio n intends to s ubmit an applic atio n fo r F Y 17 and has req uested that we as Georgetown b e a
co-spons o r. T he Sec tion 5310 p ro gram requires (at least) 20% matc h fo r Capital projec ts and (at leas t)
50% for Op eratio ns p ro jects but d o es allow fo r in-kind and the use o f Trans it Develo p ment Cred its (TDC)
to fully offset any loc al match requirements . The C ity and F aith in Ac tion have c o mp leted a preliminary
ap p licatio n. By p artnering with the City o f G eo rgeto wn, Faith in Actio n is ab le to ac cess regionally
available TDCs and meet the loc al match req uired; eliminating the need for a c as h match and provid ing
acc es s to more than $100K in grant funding.
This grant applic ation wo uld facilitate FIAG's p artic ip ation in the City led Trans it Wo rking Gro up and
p ro vide fo r mo b ility management fo r the coo rd inatio n, sc heduling and adminis tration of s upportive
trans p o rtation s ervices fo r senio r adults. This p ro gram is designed to s uppo rt FIAG’s service p ro vision
and volunteer rec ruitment effo rts. FIAG is req ues ting funds to s upport a portio n o f s taff salaries fo r a
mo b ility c o ordinator (90%) and a volunteer c oordinator (50%) as well as c omputerized s ched uling s ystem
and marketing expens es . T he mo b ility c o o rdinator acts as the service s cheduler point o f c o ntact entering
rid e reques t into an o nline datab as e for volunteer d rivers to s elf-as s ign. Sc hed ules are monitored b y the
Mo b ility Co ordinator contac ting volunteers to ens ure all rid es are as s igned. C urrent p ro visions of FIAG is
a limit of two rid es a week per client. The Mo b ility C o o rd inato r role invo lves awareness o f mobility o p tions
and coordinatio n to imp ro ve acc es s to transp o rtatio n s ervic es for s enior adults that have a need fo r more
than the two rides . As a vo lunteer trans p o rtation agenc y, it is vital to have a dedic ated Volunteer
Coordinator to foc us o n volunteer retentio n and rec ruitment. At the end of May 2016, F IAG had 508
enrolled clients with an average o f 3-4 new c lients enrolling weekly.

Dear Transportation Policy Board Members,
The City of Georgetown has submitted an application for the Federal Transit Administration
(FTA) FY17 5310 call as a co-sponsor with Georgetown based Faith in Action. This grant will
facilitate Faith in Action's participation in a Transit Working Group that includes the City, Capital
Metro and the Georgetown Health Foundation and provide for mobility management to
coordinate, schedule and administer supportive transportation services for the 500+ senior adults
currently enrolled in the program.
Faith in Action Georgetown (FIAG) through volunteer drivers provides support services at no
charge to non-driving seniors, age 65+ who live in their own homes in Georgetown including the
ETJ. FIAG's primary service is reliable, door-through-door assisted transportation to medical
appointments, grocery stores and personal errands with the ultimate goad to support senior
independence and quality of life providing access to healthcare and nutrition.
FIAG’s 2015 service achievements are impressive and 2016 promises to exceed total rides by
2.5%:
On average, 3-4 new clients enroll each week.
In 2015, volunteers contributed 6,097 hours equating to $153,096.
Rich history of providing dependable reliable services for 15 years
Client Satisfaction - Program outcomes from 2015 client survey:
o 87% - services are available when they need them.
o 98% - services help increase their independence by providing them the ability to get
out and around town.
o 93% - services are easy to use.
o 98% - regular access to medical appointments helps them better monitor health
issues.
o 76% - that if not for Faith in Action, I would have to change my current living
situation.
We appreciate your support for this critical service provider and for your coordinated efforts to
promote mobility and independent living for our residents.
